Caregivers Connections Support Program

Our Caregivers Connections Support Program is a wellness and resource program for family caregivers of persons with chronic illness or disability.

Our ultimate goal is to empower and enhance the well-being of family caregivers by serving as a visible resource for: EDUCATION, SUPPORT, ADVOCACY, RESPITE BREAKS, REFERRALS, and CONNECTIONS with other caregivers.

Individual and/or family sessions with professional staff are available and provided in any setting – home, office, hospital, nursing home, etc.

Qualified volunteers are available for practical assistance, companionship, support, errands, transportation, and brief respite breaks.

“Time Out” is a bi-weekly group support program, facilitated by our Palliative Care Counselor, which provides opportunities for education, support, and socialization with other caregivers. Meetings are held on the first and third Friday of the month from 1-2 pm via ZOOM until further notice.

“De-Stressing to Cope” is an educational program provided in groups or one-on-one which focuses on specific topics of caregiver issues and stress and which teaches aspects of positive self-care and techniques for coping.

Coaching for Caregivers provides one-on-one consultations with our Nurse Practitioner on any aspect of care, individualized assessment of needs, and individualized plans for support.

A lending library of caregiver related topics is available in our office and through materials we’ve donated to the Saltmarsh Senior Center and the Nantucket Atheneum.

Education is integrated throughout all services and includes: the importance of self-care (mind-body-spirit); stress management; communicating with health care staff; medication management; how to provide personal care of your loved one; advance care planning; family interactions; assertiveness; and much more.

Caregivers Connections is partially funded through a Title IIIB grant of MA Executive Office of Elder Affairs and the Federal Administration for Community Living. All services are provided free of charge. However, donations are sincerely appreciated!
