Adult Community Day Care (ACDC)
Provides a supported day program three days per week.
Tuesday, Thursday, and Friday from 8:30am until 2:00pm in Father Griffin Hall at 15 Cherry Street.
Elder Services may provide funding for eligible clients.
Provides regular periods of respite for participants’ families and caregivers.
Designed to enhance the physical, emotional, and social well-being of senior adults, especially those who are socially isolated due to physical or emotional limitations. The staff is trained to help those who may need some assistance in self-care, such as stroke victims, people with Alzheimer’s, Parkinson’s, and other age related limitations.
Alzheimer’s Services of Cape Cod & the Islands
Provides information, education, and support for people who have Alzheimer’s disease or a related dementia and the families and professionals who care for them.
Alzheimer’s Support Group
Held on the last Friday of each month at 1 pm at Our Island Home.
A program specifically for people with memory impairment and their care partners. Registration is required. Held on the 3rd Monday of each month from 10:30 to noon at the Nantucket Public Safety Facility.

Elder Services of Cape Cod & the Islands
A central source of information and services enabling elders on Nantucket to enhance their quality of life and maintain their independence. Programs address the interests and changing needs of older adults. Whether recently retired and seeking a volunteer opportunity or a senior needing nursing home care, Elder Services provides the access, service, and advocacy needed. Also the federally designated Area Agency on Aging (AAA) and the state designated Aging Services Access Point (ASAP) as well as the Aging and Disability Resource Consortium (ADRC) for Nantucket.
115 Main Street. A comfortable and caring home for older men and women who cannot maintain a home on their own. The applicant must be ambulatory and in reasonable good health and capable of participating independently in activities of daily living as the Homestead does not provide nursing care. 144 Orange Street. Apartment housing for persons aged 62 years and over who are handicapped or disabled and have low to moderate income requirements. Call to request an application to be preapproved and to be put on a waiting list.

A special fund which helps defray the costs of transportation and lodging for patients who must travel off-island for cancer treatment.
Nantucket AIDS Network (NAN)
NAN is committed to providing compassion and care for persons living with HIV/AIDS and/or Hepititis C in the Nantucket Community. NAN provides an accessible, confidential, client-centered system of prevention and case management; peer-based youth education and training; and community outreach and partnerships.
Nantucket Behavioral Health Service
A licensed mental health clinic with a multidisciplinary staff incorporating psychiatry, psychology, social work, and nursing which offers individual, family, and group sessions for adults and children.
Accepts insurance and has a sliding fee scale for private pay clients.

A full-service certified home health care agency committed to providing the residents and visitors of Nantucket exceptional service through individualized care plans utilizing a multidisciplinary team of skilled nurses, therapists, and health aides. A program of the Vineyard Nursing Association.
Our Island Home
A 24-hour elderly nursing care facility licensed to provide Level II and Level III nursing care to 45 residents. Our Island Home services also include rehabilitation program, social services, and activities programs.

A Nantucket community planned around the preferences of older adults, aged 62 years and over. Located on 20 acres off of South Shore Road. A full range of services is provided for a continuum of care for the elderly including 24 hour staffing, on call support, and Assisted Living facilities.
Your Island Ride
Year-round transportation to persons with disabilities and elders 60 years of age and older through the Nantucket Regional Transit Authority (NRTA). The service operates on an advanced reservation system and is door-to-door. All clients must be certified and determined eligible per ADA guidelines.
